Refactored to use GP_Plugin as the base PHP class.
Fixed issue where List field Add/Remove buttons were visible if using the Gravity Forms Theme Framework. Fixed issue where selected read-only checkbox values are not preserved after editing an entry with GravityView.
Fixed an issue with read-only Time fields not submitting the AM/PM values.
