* Tweak - You can set the cart items default handling to either pickup or ship also when using the "Automatically group cart items into as few packages as possible" option
* Tweak - When using "Automatically group cart items into as few packages as possible" checkout mode, do not show a toggle to change package handling from pickup to shipping if no other shipping methods are available for the package than pickup
* Fix - Address a pickup appointments issue where the lead time is set to zero days and the next day in calendar is a public holiday which could become available to customers for scheduling a pickup
* Fix - If Cash on Delivery is enabled exclusively for Local Pickup Plus, orders without items for pickup shouldn't have Cash on Delivery available at checkout
* Misc - Removed support for WooCommerce 2.5
